• Prior to making funeral arrangements, you will need the following information:
  
  1. Full name of the deceased
  2. Date of birth
  3. Place of birth
  4. Social security number
  5. Residence address
  6. Spouse's name (maiden name)
  7. Father's name, mother's maiden name
  8. Occupation
  9. Place of burial or disposition
  10. Discharge papers, if veteran.
  11. Insurance papers and/or insurance information
  12. Clothing and/or jewelry for the deceased (including all usual undergarments)
